Q. What information is required for a biosafety application?
A. Each biosafety application must include the Application for Biosafety Permit and a Biosafety Resume for Authorized Personnel form for each person handling the material. If the proposed work uses a biohazardous organisms with a risk factor >1, also include an Exposure Control Plan. Please attach any supporting documents (eg., ATCC documents) to the application.
If your project is a renewal, please include a reference copy of your original application.
Q. Where are the forms located?
A. The forms are located on the website under the Biosafety section of "Ethics & Safety"
Q. How many copies of the proposal do I have to submit?
A. Include a signed original and ten copies of each document. Copies can be double sided.
Q. When does a proposal have to be submitted?
A. Submissions to the Biosafety Committee must be received by the 15th of the month for review at that month's meeting.
Q. If I have more than one project, can I submit them on one set of forms?
A. No. Each project requires its own set of forms.
Q. How often does the committee meet?
A. Once a month.

Q. What do I do if a new person joins the lab of an approved project?
A. The researcher should submit a letter to the Biosafety Committee which indicates the project name, the approved protocol number, the name of the new person and the position of the new person. The letter should also enclose the biosafety resume of the new staff member (which can be found in the
Biosafety Forms section of the website). The letter and resume should be submitted to the Office of Research and Development, Kelley 236, UPEI, C1A 4P3
